Documents required for patient admission

 

1.Provide admission to the admission unit by the treating physician or emergency physician

3. Completion of the consent form by the patient or his / her companion (for persons under 18 years of age, legal guardian) requiring informed consent from surgery or treatment is mandatory.

4. Issuance and receipt of the accompanying card (upon request) by the acceptance officer

5. Provide the patient's correct telephone and address at the time of admission to call when necessary.

6. For all victims of traffic accidents and injuries, a crash report or emergency report 115 is required.

 

Essential information for admission and hospitalization

 

1.It is necessary to verify the information contained in the acceptance sheet and identification bracelet by the patient and his / her companion.

2. Patient admission is performed 24 hours a day from 7 to 22 hours in the primary admission section and from 22 to 7 days after the emergency admission.

3. It is forbidden for people younger than 12 years to be admitted to the hospital for observation of the safety and infection requirements, so avoid even bringing these loved ones as far as possible.

4. At the time of admission of the patient, upon request of the patient or with the patient's companion, a sanitary bag including (slippers, paper towels, glasses, etc.) shall be delivered.

5. Patient appointment hours at this hospital are from 15:00 to 16:00 daily.

6. Deliver your precious and valuable items to your relatives while you are hospitalized. Patients of other cities, will hand over their valuables to the fund unit and receive receipts

7. The patient should wear hospital-specific clothing during hospitalization and refrain from wearing personal clothing.

8. Coordinate with your ward clerk as soon as possible after admission to complete your insurance.

9. If you need to copy the case papers, it is possible 72 hours after the permission time.

10. A letter of application from the relevant center is required to file the case for supplemental insurance.

 

Essential information on clearance time

 

1. The patient is discharged from the ward upon the order of the treating physician and after completing the patient's clinical record.

2. After the patient's file is completed by the doctor and the nursing system, the secretary is registered in the system and then sent to the discharge unit.

3. After estimating the costs, the final case is completed and the patient can follow up with the patient from 11am to 5pm.

4. If your doctor gives you a discharge order after 4 pm, you can go to the discharge unit at least one hour after that. Vacation days are from 11am to 1pm.

5. If the patient is unable to pay for his / her expenses, he / she should consider the necessary regulations with the recommendation of the support unit.

6. Patient's instruction sheet and file summary are given to the patient at the time of discharge by the shift officer.

7. Refer to the hospital department if you need to duplicate medical records 72 hours after discharge by letter or application center
